Description

The Quality Improvement (QI) Nurse supports the organization's Quality Improvement, Population Health, and Patient-Centered Medical Home (PCMH) initiatives through clinical quality activities, preventive care outreach, care gap closure, quality data validation, and coordination of population health programs. Working collaboratively with providers, care teams, QI Coordinators, and the Quality Manager, the Quality nurse assists with improving UDS, HEDIS, CMS Star, and payer quality measures while supporting regulatory and accreditation requirements.

Qualifications/Requirements
  • Clear active Florida license as an LPN or RN.
  • Skills in providing health screenings, preventive care, and chronic disease management.
  • Ability to work effectively with a diverse multidisciplinary team of health care professionals and support staff.
  • Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work.
  • Excellent internal and external customer service skills.
  • Knowledge of adult learning methodologies and ability to engage patients in improving their health through education and support.
  • Computer skills including Microsoft Office applications.
  • Must demonstrate personal and professional commitment to the mission of the Heart of Florida Health Center and its Mission Statement and values.
  • Preferred: Primary care experience and certification or specialized training in diabetes education.
Essential Functions
  • Identify patients due or overdue for preventive services and chronic disease management using quality reports, patient registries, and the electronic health record.
  • Conduct patient outreach, education, and follow-up to improve preventive care, chronic disease management, and care gap closure, including providing scheduled Quality nurse visits focused on preventive services, chronic disease support, and patient education.
  • Support organizational quality initiatives to improve UDS, HEDIS, CMS Star Ratings, PCMH, and other value-based performance measures.
  • Review medical records and collaborate with providers to improve clinical documentation, quality outcomes, and regulatory compliance.
  • Participate in quality improvement projects, including data collection, workflow improvement, implementation of interventions, and monitoring outcomes.
  • Participate as a member of the Chronic Care Management (CCM) team by supporting patient identification, enrollment, care coordination, and program activities.
  • Serve as the designated Coordinator and facility contact for the Diabetes Self-Management Education and Support (DSMES) Program, coordinating referrals, monitoring participation and outcomes, maintaining required documentation, and supporting accreditation activities.
  • Support Transitional Care Management (TCM), and other population health initiatives as assigned.
  • Collaborate with providers, care teams, and organizational departments to improve patient outcomes and quality performance.
  • Assist with quality reporting, data validation, audits, and regulatory or accreditation activities.
  • Maintain accurate documentation and comply with organizational policies, HRSA requirements, NCQA PCMH standards, HIPAA, and other applicable regulations.
  • Participate in committees, meetings, outreach activities, and continuing education as assigned.
  • Perform other duties as assigned.
