Schedule: Every Friday | 5:30 AM – 5:30 PM

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide one-on-one in-home care for a medically fragile 1-year-old pediatric patient in Canal Winchester, Ohio. This rewarding opportunity allows you to make a direct impact by delivering personalized care in a supportive home environment.

The ideal candidate is dependable, patient-focused, and experienced or comfortable working with tracheostomy and G-tube care.

Responsibilities

• Provide skilled nursing care in the patient’s home 
• Monitor respiratory status and provide trach care as needed 
• Administer G-tube feedings, medications, and site care 
• Perform suctioning and airway management 
• Monitor and document patient condition and report changes promptly 
• Follow physician orders and individualized care plans 
• Maintain accurate and timely nursing documentation 
• Support a safe, nurturing, and family-centered environment 

Qualifications

• Active Ohio Licensed Practical Nurse (LPN) license 
• Current CPR certification 
• Reliable transportation and dependable attendance 
• Strong communication and clinical assessment skills 
• Compassionate, patient-centered approach to care 

Preferred Experience

• Pediatric nursing experience 
• Experience with tracheostomy and G-tube care 
• Home health or private duty nursing experience
